(Based on Link having full hearts at the beginning of the game) But one thing that's weird is how certain returning NPCs don't recognize Link. At all.

This is probably honestly the worst story in 3D Zelda, it just doesn't leave a good taste in the mouth, and I think this is because it's a direct sequel that presses reset, while gameplay wise it's VERY clear you're playing the sequel.

Just remember that Zelda is never older than 16. In the Oracle duology, Link is probably like 13 or 14, at the oldest. Din and Nayru are both clearly taller than the kid, who definitely is still growing. If we accept the Hyrule Historia timeline, that means that Link was even younger in A Link to the Past.
