Like the 7 Habits book says, it always helps to begin with the end in mind; it is a risk to do fellowship just because you reach your last year of residency and don't know what that end is.

But with that said, some of the top residency programs have a minimum GPA requirement of 3.5 and class rank in the top 25% so both are certainly important.
