You need to have it out to make use of it, and you need to be within reach of the target. It is a good Implement to have with the Weapon to have a little extra Temp HP. No class can use Constitution as a primary stat so it'll always be a bit below the max of another stat but you can get it 22 easily enough. If you really wanna go beefcake, then orc with Hold Scarred Heritage and Animal Instinct Barbarian makes sense.
