First you need to get to the Altus Plateau and go to Perfumers Grotto. You can either use the Dectus Medallion to get there early or use the flying horse glitch. You just need to grab the grace in there. Second you need to do the flying horse glitch and fly to the forbidden lands and the Grand Lift of Rold grace.

I've managed to get into the capital using the Perfumers Grotto wrong warp and im up to Margott and its (finally) occurred to me that Melina gives the Medallion to get into the Mountaintop after the Margott fight.You can only use a glitch. Speedrun has already use it. I'm pretty sure there's an alternative way to get there without progressing through the story. They have put up an infinity of portals and passages that allow you to explore the whole map by killing a couple of bosses. I doubt they've cut out a slice like the mountains of giants.But to reach there, you will have to take a long path. From Zamor Ruins, go northeast until you reach Freezing Lake. From there go southeast toward Spiritcaller's Cave. From there, go south to Giant's Gravepost. Then go southeast to reach the Giant Conquering Hero's Grave. The path is marked with red lines in the picture below.
